Ye, a Clerckship done in the US carries far more value than an Observership.This because a Clerckship is a pure hands-on experience.An Observership is not in any way close to that and one mostly ends up merely watching in the wards,and attending rounds rather than actually taking care of the patients.Also, the LORs which one gets from them matter a lot.
It is actually the research done in the US which helps.But, if yur research is unique and lead to some publications then it may be given some importance by the program directors.

If u can do research during internship then that is too good.Get something unique published.It will strengthen yur CV.

Yes, definitely it is.This because alongwith being a US experience it is a part of yur under-graduate medical education and is easy to get.One can do it in any of the top hospitals in the US.
An externship is done after graduation and is definitely a hands-on experience but is certainly difficult to get as one is not usually allowed to touch patients in the US after one has graduated.
So, go for the easier alternative i.e., a clerckship or an elective in the US.
