There r thousands of applicants all over the world who come to the US in order to take the USMLE Step-2CS test.Most of the people who apply for a visitor B1/B2 in order take this test get it.
However,it also depends on the country where one is applying.In some countries there may be some security concerns,etc and the visa may not be issued in great numbers.
All in all, visa applications of only a very few applicants get rejected.So, as long as one has a proper Visa Letter from the ECFMG,Scheduling Permit,Test Date Confirmation,and various other documents and one satisfies the visa officer with his/her answers then he/she will usually have no difficulty in obtaining a single or a multiple entry visitor visa.

Speaking from personal experience, you may be required to pass both USMLE Steps 1 and 2 CK before you are allowed immigration for the purpose of taking Step 2 CS.

Although it is not essential ,but I have seen visa officers asking for USMLE Step scores when one applies for a visitor visa to the US for taking Step-2CS.But, if someone is coming to the US for Clerckships or Rotations then he/she can take the Step-2CS on the same tour.
