U can even apply for Surgical Fellowships directly after completing yur GS residency at AIIMS.The reputation of yur institution will definitely help u.But, even for pursuing a fellowship in the US u need to do all the USMLE steps.But, do remember one thing that u cannot practice in the US after just completing yur US fellowship.In order to practice in the US u need to do a residency there as well.
The fellowships r also competitive to get into,because u will be competing with those who will have done their residency in the US itself.In addition,it will be very tough to get into the most competitive fellowship branches.But, u still can give it a try,and I think the reputation of yur institution will give u an edge over others.Also, u will be having a lot of publications in international peer-reviewed journals by the end of yur Surgery residency.
As for as I now getting into an MCh program in India is also very competitive.So, I think for u the best wud be to take yur USMLE steps whie u r doing yur Surgery residency.I feel it won't be difficult for u to score high on them.After u r done with them by the end of yur residency,u will have both options in hand.
