USMLE Step 1 - what should the doctor do?
what should the doctor do?
bingousmle - 11-25-08 13:36
a doctor overhears the conversation of two juniors in the cafeteria where they are discussing with each other a case of a patient where the patient with known hypersensitivity to a drug develops an allergy because of their mistake, and are afraid of being sued. what should this doctor do?
ignore the talk/advise them to report this as an adverse drug reaction/go and see the patient and review the notes/tell them discussions about patients should be kept private/pay his bill and go away/ask the details so that he should not commit this mistake

Re: what should the doctor do?
pankyash - 11-25-08 13:51 advice them to report as adverse drug reaction..
#2
Re: what should the doctor do?
bingousmle - 11-25-08 15:10 i think that would not help much since they were already aware that the patient had a history of hypersensitivity , and that this mishap occured due to their negligence..

Re: what should the doctor do?
sackatdoc - 11-25-08 23:43 hi, bingo, how are you??? i guess any over heard information should not be used for taking any action, not even for walking up to the people he was busy overhearing and advising until and unless there is a question of life and death, which isnt the the case over here, whats the answer???

Re: what should the doctor do?
bingousmle - 11-26-08 00:54 but you are a senior , and definitely you need to do something , since it is a question of ethics.. as far as the genuineness of the talk is concerned , we need to believe them...
again its a modified NBMEquestion..

Re: what should the doctor do?
sackatdoc - 11-26-08 10:11 The need to do something will arise when things are getting out of control or there is persistent threat which I assume is not, there was a allergy, what kind and to what extent things got worse has not been mentioned so I THINK we should not over react to the facts and top of all that other choices arent all that appealing.
#8
Re: what should the doctor do?
bingousmle - 11-26-08 10:33 how about the discussion about patients should be kept private??
#9
Re: what should the doctor do?
sackatdoc - 11-26-08 11:28 as these doctors are not disclosing the information about patient intentionally, it has been overheard, so it is the problem of the doctor who is busy hearing the other peoples conversations or he just happened to hear, so first we need to know the exact language of the question and then we will be good to comment.

Re: what should the doctor do?
bingousmle - 11-26-08 14:10 the conversation was something like.."how could you do this blunder?you knew Ms Wilson was allergic to penicillin and that the antibiotic cross reacts with penicillin . now she has developed a rash , i hope she does not develop respiratory problems

Re: what should the doctor do?
sackatdoc - 11-26-08 19:30 okay, so we get all the details, still things are too tricky, and there is a risk of death or serious complication that may arise due to negligence, so the answer may be something like asking doctors to inform the patient or authorities about the mistake and that action could help them avoid things getting worse.

Re: what should the doctor do?
bingousmle - 11-26-08 23:30 agreed.but if you look it in a subtle way, i think the question could be about the doctors keeping the confidentiality of patients condition as well..

Re: what should the doctor do?
kingofhearts - 11-27-08 09:31 maybe could it be to ask the details so that he does not repeat the same mistake and is more careful if has to come over such a case ......

Re: what should the doctor do?
bingousmle - 11-27-08 10:06 no i dont think so, the patient was a known case of penicillin hypersensitivity, and the mistake was a negligence.. also i think the question is not about a CME..

Re: what should the doctor do?
bingousmle - 11-27-08 10:34 i am not sure , that is why i have put this here , so that we can reach at a sound and logical and the most appealing answer, what i think is that the privacy of the patients disease condition should be respected...

Re: what should the doctor do?
sackatdoc - 11-27-08 12:43 no i think you are not thinking on right lines, coz the question of privacy comes when things are of that sort, like HIV status, paternity issues etc and if they were told aloud in public and the matter comes out in open, whereas here how to handle NEGLIGENCE, that is what is being tested, think form my perspective, and you know already we are open to discuss.
