If u want an H1b visa then u must take Step-3 before starting yur residency.
But, keep in mind the fact that u must try to score high(at least in 80's) if u wanna take Step-3 before residency because,nowadays, residency program directors also care about step-3 scores.
Yes, one can apply to J1 programs even after passing Step-3.
