Which one is better, observership in a university hospital which is free or externship which I don't know exactly where and cost $$$?
What do u know about Americlerkships, I have a freind who supposed to start an externship with them ina teaching hospital ina month but till now he don't know where, and it costs about $2500/month?

Externship is a hands-on experience and in observership one is not usually supposed to touch patients.But, finally it is actually the LOR which actually matters.An LOR from a big university hospital may be given much more weight.If the person under whom u r doing an observership or an externship is well known in his/her field then that will also count.
It will better first to talk to the university which is offering u the observership.Talk to them about how much clinical work will u be allowed to do there.If they r also willing to allow u to take histories and do physical exams under supervision then that wud be fine.
However, if money is not a constraint then u can go for an externship as well.But, the fees they r demanding r really huge.Do talk to someone who has already done an externship thro' them.Check whether they r putting u with some private practitioner or really in a teaching hospital.
In a nutshell, take an informed decision based on these facts.
